Haryana NHM 787 MLHP-CUM-CHO Vacancy 2022
Information : Haryana State Health Society is invited applications for 787 posts of Mid-Level Health
Providers-cum-Community Health Officers (MLHPs-cum-CHOs) on contract basis under National Health Mission (NHM) for providing Comprehensive Primary Health Care Services at “Sub Center level Health & Wellness Centers” (SC-HWC) being upgraded under Ayushman Bharat in 18 Districts. - Date, Time & Venue: For Document Verification & Counseling and Signing of for Surety Bond : 02 to 17 May 2022 9:00am to 5:00pm at NHM SHQ Panchkula.
Application Fee Detail
- UR (General Category): Rs.100/-
- BC-A/BC-B/EWS/SC/ PWBD/ FF/ ESM/OSP : Rs.50/-
- Fee Pay Online with any payment mode.
If any candidate submit more than one application in different District/categories, she/he has to pay fee for each
application separately and her/his candidature shall be considered only against the application for which she/he appeared in the OMR Based Exam. Requisite fee is required to be deposited online only, and no cash/physical fee is allowed.
Essential Academic Qualifications
- Candidate must have passed BAMS or B.Sc. Nursing / Post-Basic B.Sc. Nursing Degree and completed their Six Months MidLevel Health Provider’s Bridge Course Programme on Certificate in Community Health for Nurses (BPCCHN) from IGNOU.
- BAMS or B.Sc. Nursing / Post-Basic B.Sc. Nursing Degree Holders.
Important Note regarding Essential Academic Qualifications
(i) The candidates with academic qualifications as per above information are required to provide Valid Certificates in prescribed formats only to ensure their eligibility for Six Months BPCCHN from IGNOU or any State University / Institution.
Upper age limit as on 01/01/2022
Minimum Age : Candidate must be more than 18 years.
Maximum Age : 42 Yrs.
Reservations & Relaxations for SC/ST/OBC/PwD/EWS will be as per Govt. instuctions. A candidate for this examination attained age of 42 years as on 01-01-2022.
i) Upto five (5) years Relaxation for SC candidates in MLHP-CUM-CHO Vacancy.
ii) Upto three(3) years Relaxation of OBC candidates in MLHP-CUM-CHO Vacancy.
iii) Upto fifteen(15) years Relaxation for SC-PWD candidates in MLHP-CUM-CHO Vacancy.
iv) Upto thirteen(13) years Relaxation for OBC-PWD candidates inMLHP-CUM-CHO Vacancy.
v) Upto ten(10) years Relaxation for OC-PWD candidates in MLHP-CUM-CHO Vacancy.
Important Instructions to Candidates
a) Candidate must be read the complete advertisement carefully and ensure that he / she fulfills the eligibility criteria of the post which is mentioned in the advertisement.
- The candidates applying for the posts of MLHP-CUM-CHO should ensure that they fulfill the prescribed eligibility conditions for the above posts. Their admission at all the stages of examination viz. exam, Interview for which they are admitted .

- At the time of entering in exam and Interview ,Biometric Verification is mandatory. So those, All Candidates must carry out Aadhar Card and any relevant Documents for Documents Verification at all level.
- Candidates are allowed to enter in exam till the reporting time of exam. The late incoming person will not be able to enter in interview examination after reporting time.
- No Use of electronic devices at time of interview.
- Candidate must be bring a black or blue pen.
- After the commencement of any request or appeal, one is permitted to leave the Interview till the end of the Interview will not be permitted to any candidate.
- No TA/DA will be payable to any candidates.

- Applications must be submitted in online mode which is prescribed by National Health Mission.
- In the offline Application Form, Candidate fill all required details which is mention in performa for applying online form.
- If the proper photograph is not attached by a candidate in online application form, his candidature will be cancelled or Rejected. Any of photographs which are acceptable/ not acceptable at the time of Interview or Document verification.
- Before last date submit this application, Candidate must pay fee and submit form.
